## About

Collaborative Aotearoa is a charitable trust and national network of primary healthcare organisations in New Zealand (Aotearoa). Our purpose is to foster health and hauora equity for all people within Aotearoa.

We bring together leaders from PHOs (Primary Health Organisations), general practices, and community health providers committed to building an equitable future through transforming health and wellbeing outcomes for all New Zealanders.

## What We Do

- **Model of Care Development**: Supporting healthcare organisations to implement the Health Care Home model and enhanced models of care
- **Collaboration & Networking**: Connecting over 100 healthcare organisations across New Zealand for shared learning and best practice
- **Resource Sharing**: Providing a library of healthcare resources, guidelines, templates, and research
- **Events & Webinars**: Hosting conferences, workshops, and peer learning sessions
- **Consultancy Services**: Fee-for-service support for healthcare organisations on projects, capability building, and system change
- **Research & Evaluation**: Supporting healthcare research and evaluating care delivery models
