This interim report has been compiled as a summary of 21 evaluations available to date on the implementation of the Health Care Home model in practices across Aotearoa, New Zealand. Overall, the model is seen as having a positive impact on both staff and patients/whānau. The key findings have been grouped according to the key values of the reviewed Health Care Home model care (HCH Collaborative, 2020)
Collaborative Aotearoa Te Ara Ako Study Tour Report v1.0

In 2023, The Health Care Home programme moved into its eighth year at Tū Ora Compass Health, implementing and embedding the model of care within primary health across the Wellington and Wairarapa regions. In this publication, reflections share further insight and depicts the positive impacts of the Health Care Home (HCH) model through practice stories and the patient voice.
