Pinnacle Midlands Health Network

Pinnacle is the parent in a group of not-for-profit primary care focused organisations. We’re a network of forward thinking general practices that manage the healthcare of nearly half a million people enrolled with 86 practices in Tairāwhiti, Taranaki, Rotorua, Taupō-Tūrangi, Thames-Coromandel and Waikato. The Pinnacle Health Care Home journey began in 2008. We created our own ‘model of care’ sensitive to the New Zealand context that would future proof general practice from the future challenges of rising demand and an ageing workforce. Our first HCH practices were established in April 2011 and we now have HCH sites across our network ranging from small to large, and urban to rural.

Mahitahi Hauora
Neighbourhood Healthcare Homes is a collaborative programme funded by Northland DHB and Te Kaupapa O Mahitahi Hauora. The regional programme is led by the improvement team at Mahitahi Hauora. The programme has evolved over the past four years with the current model aligning with the HCH Pae Ora MoC and Whakamaua the Māori Health Action Plan. The model is offered in 15 general practices with a combined population of 109,478 with 34% of our population being Māori. This equates to over 60% of the total enrolled population in Northland. The model of care in Northland is strong and well established allowing the programme to evolve and dive deeper into providing equitable health outcomes.

Whanganui Regional Health Network
Whanganui Regional Health Network
Whanganui Regional Health Network (WRHN), formerly known as Whanganui Regional Primary Health Organisation, was established in July 2003 and is registered as a charitable trust. There are 11 general practices under WRHN, situated within the city and also based in the rural communities (GP Fee Schedule). As of 1 January 2017 our enrolled population totaled 55,993; of which 15,884 are Māori and Pasifika and 11,206 are non-Māori/PI that live in a Deprivation Five area, therefore 48% of the WRHN enrolled population have known inequalities in health due to ethnicity or socio-economic status.
As a Primary Health Organisation (PHO) it is our role to provide essential primary health care services to those people who are enrolled with us. PHOs bring together doctors, nurses and other health professionals in the community to support the health needs of their enrolled populations. When your doctor decided to join a PHO you would have signed a consent form and become an ‘enrolled PHO patient’. PHOs can now do more things to keep people healthy, by improving access to health services and connecting with people in the community who are missing out or have poor health. PHOs play a part in helping reduce acute demand pressure on hospitals, by better managing chronic conditions and proactively supporting high need populations.

THINK Hauora
THINK Hauora
THINK Hauora is a not-for-profit charitable trust, that provides primary healthcare services to the MidCentral DHB population. Primary healthcare services are managed and delivered in the community through the PHO. THINK Hauora has recently joined the Health Care Home collaborative and is working to deliver better access option for 60,000 people in the Horowhenua, Tararua and Manawatu areas.

Marlborough Primary Health
Marlborough Primary Health
Marlborough Primary Health contracts health professionals and organisations in the community, such as General Practices, to deliver quality primary health care services to an enrolled population of approximately 46,600 people. There are currently eleven General Practices operating in the Marlborough district.
Marlborough PHO and Nelson Marlborough Health are working in partnership to introduce and deliver the Health Care Home programme to the Marlborough district. Our focus is on improving access and equity, strengthening coordinated care for those with complex needs, and strengthening GP sustainability through implementation of the Health Care Home programme.

Te Awakairangi Health Network
Te Awakairangi Health Network
Hutt Valley District Health Board, Te Awakairangi Health Network and Cosine PHO work as an alliance and have collaboratively developed and funded the Hutt Valley Health Care Home programme in the district. In addition to the National Health Care Home model of care requirements, the Hutt Valley programme also focuses strongly on equity, consumer co-design and community integration.

WellSouth Primary Health
Southern Health Care Home has 29 practices at various stages of HCH. 60% of total Southern registered patients are in an HCH practice, and around 63% of Māori and 69% of Pasifika people. NBPH is working in alliance with Marlborough PHO and Nelson Marlborough Health to introduce the local Health Care Home programme to the Top of the South region for improved primary care access and options. Western Bay of Plenty PHO provides primary health care services to 26 general practices across the Western Bay of Plenty sub-region, covering an enrolled population of over 165,000 patients, and a further 3 practices within Whakatane covering an enrolled population of almost 21,000.

Aifou, 38 FemaleAifou called her practice to book an appointment for a suspected UTI. Within minutes, her doctor had called her back and was able to assess her needs over the phone. The doctor then booked Aifou to see the nurse that same morning, using a standing order to enable the nurse to prescribe antibiotics if necessary. During her appointment, the nurse also notices that Aifou is due for a cervical screening and a mammogram, and books these in with the appropriate service providers with Aifou present.
