Model of Care Pulse Check
Everything you need to know about the annual Model of Care Pulse Check survey.

No, this is an optional pulse check, not mandatory reporting.
Previously (2023–2025):
- Data was collected quarterly via Excel, submitted by PHO Collaborative leads
- Based on 13 Health Care Home Model of Care questions, mostly Yes/No responses
- Some responses were inconsistent, with gaps where questions were difficult to answer
Now:
- Moving to annual data collection
- Transitioning from Excel to an online survey
- Shifting from ‘reporting’ to a pulse check survey approach
- Including more quantitative questions (instead of Yes/No only)
- Introducing a practice-direct option, alongside PHO-supported submissions
The annual pulse check provides a clear picture of how Models of Care are evolving across general practice. It helps:
- Practices reflect on their current model
- Collaborative Aotearoa understand what’s working (and what’s not)
- Identify where additional support or resources may be needed
- The pulse check will be run annually, with a minimum collection period of three months
- The current survey period is 1 April 2026 to 1 June 2026
- The survey link is available via the member portal (resource library) as a pinned resource
- It will also be shared through PHO networks, national communications, and social media channels
All general practices (not limited to HCH/NHH/Hikitia practices). Practices can complete their own response, or some PHOs have opted to coordinate and submit responses on behalf of their network. Practices affiliated with Health Hawke’s Bay and the Mahitahi network may not be required to participate.

This pulse check is focused specifically on Models of Care and how they are being implemented in practice, rather than patient-level outcomes or population demographics.
Ethnicity data is already collected through existing national systems (e.g. enrolment data and reporting to Health New Zealand | Te Whatu Ora), where it can be analysed at scale and alongside clinical outcomes.
To minimise duplication and reduce burden on practices, we are not asking for ethnicity data in this survey. Instead, we are focusing on understanding how care is delivered.
Equity remains a core focus of this work, and insights from this pulse check can be considered alongside existing ethnicity data held nationally.
