NHH Evaluation Report - Third Year Achievements and Reflections Aug 2020

We would like to acknowledge all of the Northland NHH (Neighbourhood Healthcare Home) practices who have worked hard over the last 3 years to implement this new model of care. Despite challenges such as GP shortages, many practices closing books to enrolments, droughts, floods and a pandemic - these practices have stayed committed to the NHH journey by working endless hours to embed processes and sustain new ways of working.
