Procon Telephone Triage Form Overview

A form has been developed and has been in use since 2017 to support practices doing telephone triage as part of the Neighbourhood Healthcare Homes initiative. The form is available for the Medtech32, Evolution, and MyPractice practice management systems. It has been updated to include decision support as to whether a patient is eligible for Covid-19 testing. In Medtech32 and Evolution, it is an Advanced Form. In MyPractice it is a web form.
